PC/LLC/LLP Corporation Registration Form for Law Offices

This form is to be used by law offices who form a professional corporation (PC), limited liability corporation (LLC), or limited liability partnership (LLP) for the purpose of practicing law. To be certified by the Indiana Supreme Court, all registrants must meet the general provisions as stated in Rule 27 of the Indiana Rules of Court. Corporation Information

Please ensure that the name of your organization is stated exactly as you wish it to appear on your Certificate of Registration.
Corporation Type:	You must supply your Corporate Type.
Corporation Name:	You must supply your Corporation Name.

Agent Information

The contact information provided should be for the attorney deemed the Agent. The provided name and email address will be used for annual renewal notices as dictated in Rule 27, Section 6 of the Indiana Rules of Court. The provided email address and password will be used to generate a new corporate account through which final registration steps and annual maintenance can be completed.

The address information provided will be used for any paper correspondence from the Office of Admissions and Continuing Education. Address provided must be a physical location. PO boxes are not accepted.
